What is Exercise Referral?
Working in partnership with Surrey County Council, Active Surrey & the NHS, our exercise referral scheme is a 12 week bespoke programme of supervised activity for the prevention, improvement and/or medial management of individual health conditions.
You can obtain a referral for exercise from your GP, Practice Nurse or Consultant for the following conditions:-
- High blood pressure (hypertension)
- Obesity (weight management)
- Mechanical back pain
- High cholesterol
- Injury rehabiltation
- Cancer rehabiltation (pre/post operative)
- Pre-diabetes, type I and type II diabetes
- Chronic pain
- Mental illness
Our referral scheme is coupled with our Live Well Exercise Programme, designed to provide bespoke, instructor led group sessions to enhance recovery, progress and improve wellbeing. Click here for more details.
What can I expect from the Referral Scheme?
You will meet a qualified exercise specialist, who will devise an activity plan with regular reviews to see how you are getting on, in order to:
- Help you achieve your best results
- Provide safe and effective exercise
- Empower you to make positive changes to your lifestyle and become more active
- Advise, motivate and support you
The activity plan is a 12 week personal exercise plan tailored specifically for you. The plan will be based on the following information you provide to us:
- Your current health status
- Any medical conditions you may have
- What you enjoy doing
- What you would like to try
- Your lifestyle
How can exercise help me?
You will have been referred on to the scheme because your GP or Health Professional thinks you will benefit from more exercise. Research shows that increasing the amount of activity you do can make significant improvements to your health, well-being and lifestyle.
The key to enjoying exercise is to incorporate fun activities in to your every day life. We will help to start you off in a safe environment and raise awareness as to what is available in the area.
How do I take part?
It's really quick and easy! Firstly, you need to get in contact with your GP who will ask you to complete a 'Patient Referral Form'. Once you've got your referral from you GP, Practice Nurse or Consultant, you can then contact one of our on-site exercise referral and weight management specialists to arrange your initial consultation. If you already have a referral (dated within 3 months) please call 01483 443322 to arrange your appointment with one of our referral specialists. You will need to bring your 'Patient Referral Form' with you on your first visit.
How much does the scheme cost and when can I attend?
The initial registration fee is £15.00. This includes a free consultation and introduction with regular monitoring over your 12 week plan from an exercise referral specialist. You will then have access to Guildford Spectrum Gym & the Live Well Exerise Programme, with the option of 2 payment methods:
1. £60 for 12 weeks - unlimited gym and Live Well exercise sessions during superivsed times (see below)
2. £4.65 per gym visit or £4-£4.50 per class on a pay as you go basis.
To ensure we have an exercise referral specialist on hand throughout your visits, exercise referral session times are:
- Monday - Friday 630am-5pm
- Saturday & Sunday 12pm-6pm
Our Cardiac Rehabilitation programme is fully supported and endorsed by Royal Surrey County Hospital who we work in close partnership with to provide ongoing exercise support and care in a safe environement following Phase 3 cardiac rehabilitation.
Once you have completed your Phase 3 referral at Royal Surrey County Hospital, you will be referred to Phase IV Cardiac Rehabilitation at Spectrum Gym. We run fully supervised and instructor led sessions with our Cardiac Rehabilitation specialist, Sandra Gates. Our Phase IV sessions run on Mondays 1030-1130am.
Once you have completed your Phase IV referral, which is specific to each individual and based on your medical condition, attednance, progress and ability to manage appropriate exercise intensity you will be invited to attend an ongoing Cardiac Rehab support session on Mondays 1130-1230 and discharged from the Phase IV programme.
Each session is £4.50 on a pay as you go basis until you have been discharged from the Phase IV programme. Once you have been discharged from the programme, ongoing cardiac rehab support sessions and bespoke, instructor led group sessions are available as part of our Live Well programme.
